Our rehab to home, Wing 5, offers assistance to anyone in the community after a major or minor orthopedic surgery, injury, stroke, cardiovascular difficulties, or an event that has left the individual unable to engage in customary routines.
Each patient is evaluated and an individualized program is recommended for recovery. The prescribed therapy is approved by the patient’s physician and adjustments are made based on the extent of the patient’s recovery. Through careful evaluation and open communication, we assist the patient to achieve a safe transition to independence.
Wing 5 offers private rooms lining a corridor that culminates in common areas: gym, lobby, dining room, and nursing area. The rooms also include cable TV and local phone service. This wing also has its own parking lot and entrance.
This wing is designed for any type of resident: dependent, independent, short term such as recovery from surgery or long term after a possible joint replacement. Our 1,000 square foot gym is home to numerous amounts of the latest therapy equipment and programs.
You will find around the clock nursing care, housekeeping, and daily activities. Our physical therapists focus on independent mobility and regaining strength & balance. Occupational Therapists promote independence with daily living activities such as bathing, dressing, eating, and home management tasks. The goal of our Speech Therapists is to re-learn cognitive skills that have been lose, muscle retraining, and use of communication devices. All therapies work with not only the residents, but also staff and family for education and training. Therapists may take the resident out into the community to practice grocery shopping or other daily living skills. They may also perform a home evaluation to ensure all is safe and accessible in the home prior to discharge.
